Job summary

A Place of Grace, Inc. in Escondido, California is seeking a knowledgeable Licensed Vocational Nurse Consultant to support health care coordination and medication-management needs for residents at our Vermel facility, a CPP-designated EBSH home.

The LVN Consultant will work with Administrators and RN Consultant to ensure medications are available and that resident appointments, referrals, health care recommendations, and follow-up care are properly tracked and completed.

Qualifications

• Must hold a current California LVN license.
• Experience with health-care coordination and medication-management is preferred.
• Excellent documentation and communication skills are required.

Responsibilities

• Coordinate and track medical, dental, psychiatric, laboratory, referrals, and follow-up appointments.
• Review physician orders, prescriptions, and discharge instructions for accuracy.
• Maintain confidential, well-organized health records and documentation.
