Job summary

A Place of Grace, Inc. in Escondido, CA is seeking a licensed vocational nurse (LVN) consultant to support health care coordination and medication management for residents at our Vermel facility.

This consulting role offers approximately 15–30 hours per month with a flexible schedule and compensation of $45.00–$55.00 per hour, depending on qualifications. The LVN will work with Administrators and the RN Consultant to ensure timely appointments, accurate documentation, and coordination of

Qualifications

• Must hold a valid California LVN license.
• Minimum 1 year experience with individuals with intellectual or developmental disabilities or similar.
• Knowledge of medication management and health-related documentation.
• Strong organizational and written/verbal communication skills.
• Ability to maintain accurate and confidential resident records.

Responsibilities

• Coordinate and track medical, dental, psychiatric, laboratory, referral, and follow-up appointments with transportation needs and documentation.
• Review physician orders, prescriptions, and discharge instructions; ensure follow-up items are documented and communicated.
• Oversee medication management activities: MARs, orders, availability, refills, pharmacy deliveries, and disposal records.
• Communicate with pharmacies and health care providers to resolve medication and coordination issues.
• Maintain current, confidential health care, appointment, medication, and physician-order records.
