Job summary

Victorian Post Acute in San Francisco, CA is seeking a Licensed Vocational Nurse (LVN/LPN) to join our clinical team and provide direct nursing care.

We offer full time morning shifts (7:00 AM to 3:30 PM) and part time evening shifts (3:00 PM to 11:30 PM) with pay range of $36.00-$38.00 per hour (based on experience) and a benefits package including health, PTO/holiday, 401(k), and career growth opportunities.

Qualifications

  • Current, active, and unrestricted California LVN/LPN license.
  • Graduate of an accredited LVN/LPN nursing program.
  • Long-term care, skilled nursing, or post-acute experience preferred.
  • Strong clinical, communication, and leadership skills with a commitment to resident care excellence.

Responsibilities

  • Provide direct nursing care and medication administration in accordance with physician orders and resident care plans.
  • Supervise and support CNAs to ensure quality resident care and compliance with regulatory standards.
  • Monitor resident conditions, document clinical updates, and communicate changes to physicians and families as needed.
  • Assist with admissions, discharges, treatments, assessments, and interdisciplinary care coordination.

At Victorian Post Acute, we are committed to delivering exceptional skilled nursing and rehabilitation services in a compassionate and resident-centered environment. We are seeking a dedicated Licensed Vocational Nurse (LVN/LPN) to join our clinical team.

Available Shifts:
  • Full Time AM (7:00 AM to 3:30 PM)
  • Part Time PM (3:00PM to 11:30PM)
Pay Range:

$36.00-$38.00 per hour (based on experience)

Position Highlights
  • Provide direct nursing care and medication administration in accordance with physician orders and resident care plans.
  • Supervise and support CNAs to ensure quality resident care and compliance with regulatory standards.

Leadership & Documentation
  • Direct daily nursing activities and ensure adherence to facility policies and procedures.
  • Maintain accurate resident records, treatment documentation, and physician orders.
  • Complete incident reports and participate in quality assurance initiatives.
  • Collaborate with nursing leadership to ensure smooth shift operations and resident-centered care.
Compliance & Safety
  • Follow all federal, state, and facility regulations, including HIPAA and infection control standards.
  • Ensure proper medication management, narcotic accountability, and treatment administration.
  • Promote a safe, clean, and sanitary environment for residents and staff.
  • Participate in facility surveys, safety drills, and required in-service training programs.
Qualifications
  • Current, active, and unrestricted California LVN/LPN license.
  • Graduate of an accredited LVN/LPN nursing program.
  • Long-term care, skilled nursing, or post-acute experience preferred.
  • Strong clinical, communication, and leadership skills with a commitment to resident care excellence.
