Cantex Continuing Care Network is seeking an LVN/LPN Charge Nurse for overnight shift at our skilled nursing location in Lufkin, Texas. The role focuses on direct patient care within licensure scope and ongoing monitoring of patient status.

You will document assessments in the EMR, administer medications, and coordinate care plans while supervising CNA staff to ensure high-quality care around the clock.

Qualifications

• Graduate from an accredited nursing school.
• Maintain patient care within LVN/LPN scope and follow policies.
• Document patient assessments and changes per procedures.
• Administer medications and treatments per order.
• Supervise CNA staff to ensure quality care.

Responsabilités

Qualifications: Responsibilities

• As an LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports Patient conditions/changes and documents in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.
• EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.

Requirements

• Graduation from an accredited Nursing School
• Valid LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse License
