Job summary

The Ohio State University Wexner Medical Center is seeking a dedicated nurse coordinator to manage post-discharge follow-up care for heart and lung transplant recipients. This role acts as a bridge among transplant surgeons, nurses, pharmacists, labs and home health services to ensure comprehensive, patient-centered care.

Ideal candidates will be a registered nurse with Ohio licensure; BSN is preferred, and transplant experience with CCTC certification is a plus.

Qualifications

• Graduate from an approved school of nursing.
• Current Ohio License.
• BSN preferred.
• Prior transplant experience preferred.
• Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.
• Flexible, sensitive and adaptable to transplant activities.

Responsibilities

• Coordinate post-discharge follow-up care for heart and lung transplant recipients.
• Serve as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies and other support services.
• Provide patient education as indicated.
• Assess patient data and collaborate with the transplant team to determine appropriate actions.
