Job summary

Ohio State University is seeking a Lung Transplant Coordinator (RN) to manage post-discharge follow-up for heart and lung transplant recipients at University Hospital. The role acts as a liaison among surgeons, physicians, nurses, pharmacists, laboratories, and home health services while educating patients and families.

The position requires an RN with Ohio licensure; BSN is preferred, and prior transplant experience or CCTC certification is a plus.

Qualifications

• Graduate from an approved school of nursing.
• Current Ohio License.
• BSN preferred.
• Prior transplant experience preferred.
• Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.

Responsibilities

• Coordinate post-discharge follow-up care for heart and lung transplant recipients in collaboration with transplant surgeons, physicians, nurses, pharmacies, laboratories, home health agencies and other support services.
• Provide patient education as indicated.
• Assess patient data and collaborate with the transplant team to determine appropriate actions.
