Job description

Title: LTSS Service Care Manager
Location: Region - 10 Broward County - member facing
Duration: 06 Months (Potential to extend)
Shift: 8:00 AM - 5:00 PM
Job Description:

Day to Day Responsibilities of this Position and Description of Project:

  1. Managing a caseload for healthcare members with long-term care needs.
  2. Monthly and quarterly member contact, including 80% travel. Remote role. Driver's license required.
  3. Conducting member assessments and documenting notes.
  4. Completing assessments with members, caregivers, or providers to gather information for care plan development.
  5. Monitoring service delivery and following up with members, caregivers, or providers through visits and calls.
  6. Authorizing and coordinating referrals for services.
  7. Ensuring provider services are delivered without gaps and identifying deficiencies in care plans.
  8. Assisting in developing informal or voluntary services to support the member’s care plan, collaborating with discharge planners, physicians, and others to ensure proper discharge and care planning.
  9. Assisting members with filing and resolving complaints and appeals.
Internal/External Groups with which the Candidate will interface:
Required Skills/Experience:
  1. 2+ years of Care Management experience
  2. Experience with long-term care
  3. Experience with electronic medical health records
  4. Proficiency in Microsoft Office
  5. Bilingual (Spanish) preferred
Education Requirement:

Bachelor's degree and 2-4 years of related experience.

Software Skills Required:

Microsoft Office, Experience with electronic medical health records

Required Certifications:

Valid driver's license
