Job description

Title: LTSS Service Care Manager
Location: Region 5 - Pinellas and Pasco
Duration: 06 Months (Potential to extend)
Shift: 8:00 AM - 5:00 PM
Job Description:

Day-to-day responsibilities of this position include managing a caseload for healthcare members with long-term care needs. The role involves monthly and quarterly member contact, requiring 80% travel; it is a remote role and requires a valid driver's license.

  • Conduct member assessments and document notes.
  • Complete assessments with members, caregivers, or providers to gather information regarding client status, support systems, and service needs for care plan development.
  • Monitor service delivery and follow up with members, caregivers, or providers through in-person visits and phone contacts.
  • Authorize and coordinate referrals for services.
  • Ensure timely delivery of provider services, identify functional deficiencies in care plans.
  • Assist in coordinating informal or voluntary services into the member’s care plan. Collaborate with discharge planners, physicians, and others to ensure appropriate discharge and care planning, as well as coordination of acute and long-term care services.
  • Assist members with filing and resolving complaints and appeals.
Internal/External Groups:

Candidate will interface with various internal and external groups involved in member care and service coordination.

Required Skills/Experience:
  1. 2+ years of Care Management experience
  2. Experience with long-term care
  3. Experience with electronic medical health records
  4. Proficiency in Microsoft Office
  5. Bilingual in Spanish is preferred
Education Requirements:

Bachelor's degree with 2-4 years of related experience required. Education preferred includes additional certifications or relevant coursework.

Software Skills Required:

Microsoft Office, Experience with electronic medical health records

Required Certifications:

Valid driver's license
