Job description

Location : Travel. This is a remote / work-from-home position, with travel required for member visits up to 75% of the time. LPN / LVN license or licensed social worker is strongly preferred. Need someone who resides in Asherton, Big Wells, Pearsall, Dilly, Cotulla, Crystal City, or Carrizo Springs area. They will cover all surrounding areas including Uvalde and Del Rio.

Requirements for this position :

• Must meet quality standards of assessments of 92% or above.
• Documentation must be completed for assessments within 3 business days.
• Required travel of 75%.
• Mileage reimbursement provided for member-facing visits.
• Travel within the MRSA West Area 3 days a week; work remotely 2 days.

SHIFT : 8am-5pm, Monday to Friday. Overtime may be required at times.

Duration : 3+ months, with a possible contract-to-hire opportunity.

Job Description :

• The LTSS service care manager will have 3 days of travel for home visits and 2 days working remotely for documentation, follow-up, and other tasks. The SC will assess members for approval or denial of personal attendant services to assist with Activities of Daily Living by completing an H2060 form in the field.
• Complete required assessments and forms, document assessments, and submit for authorization or denial of services within a 3-business-day turnaround.
• Evaluate the needs of the most complex and high-risk members and recommend a care plan for optimal outcomes.
• Act as liaison and advocate between members, families, physicians, and facilities or agencies.
• Provide education to members and their families/caregivers on preventive care, procedures, provider instructions, treatments, referrals, medications, and healthcare benefits.
• Coordinate community resources, including medical and social services, and manage service authorizations based on assessments and care plans (e.g., meals, employment, housing, transportation).
• Ensure appropriate referrals based on individual needs and facilitate access to providers, specialists, and community resources.
• Maintain accurate documentation supporting care management activities in the electronic system, ensuring compliance with guidelines and regulations.
• Provide feedback to leadership for quality improvement and cost-effective service delivery.
• Perform other duties as assigned and comply with all policies and standards.

Must haves : Experience working with the aged, blind, or disabled population for 5+ years. Bilingual skills strongly preferred.

Nice to haves : Experience in Social Services, Social Work, Care Management, or Service Coordination. Nursing / LVN / Medical education / Certification required: Bachelor's degree with 2-4 years of related experience, or LVN with similar experience. Preference for candidates with a medical or social work background and at least a bachelor’s degree.
