Job summary

You will advocate for members, coordinate with clinicians and Health Management programs, and use motivational interviewing to address barriers to care. This on-site role includes education, referrals, and assistance navigating healthcare services.

Responsibilities

• Establish and maintain trusting relationships with individuals, families, and providers to promote health, recovery, resiliency, and wellness.
• Coordinate with licensed clinicians and Health Management programs; adjust workflows to provide optimal care.
• Use motivational interviewing to encourage attendance, close care gaps, and promote service utilization; discuss health concerns with families.
• Deliver education, resources, referrals, and connections to community health services; promote new programs.
• Guide members facing complex health cases through navigating healthcare systems; may include home visits and accompanying to appointments.
• Provide culturally appropriate health education and clear instructions on using services.
• Performs other duties as assigned.
