• Coordinate and collaborate with licensed clinicians to ensure optimal care.
• Use motivational interviewing to encourage care and close gaps.
• Provide education, referrals, and connections to health-related services.
• Guide community members through complex health systems and navigate appointments.
• Deliver culturally appropriate health education and information on services.
• Perform other duties as assigned within scope and licensure.
