Qualifications

Responsibilities

• Assess member needs through comprehensive physical, psychosocial, and health literacy evaluations
• Develop actionable, individualized care plans to prevent avoidable utilization
• Coordinate with interdisciplinary teams and families to implement care plans
• Ensure effective communication across PCPs, specialists, and external partners
• Lead or participate in quality improvement initiatives and mentor colleagues
• Document all care activities thoroughly and advocate for members across the care continuum
• Perform and coordinate LTSS assessments and home/community services as needed
• Maintain HIPAA compliance and safeguard patient privacy
