Qualifications

• Clinical licensure/certification required only if state contract mandates it.
• Bachelor's in a health care field may be required in some states.
• Valid driver's license, transportation, and adequate auto insurance.
• Knowledge of community resources and service options.
• Ability to work independently with minimal supervision.
• Strong written and verbal communication skills.
• Proficiency with Microsoft Office and online portals.
• Experience with individuals with substance use disorders (state dependent).

Responsibilities

• Provides support for care management and LTSS activities.
• Collaborates with multidisciplinary teams to coordinate member care.
• Ensure member progress toward outcomes and quality care.
• Complete comprehensive member assessments within timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with care network.
• Monitor care plans and adjust as needed for effectiveness.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and advise on funding sources.
• Facilitate ICT meetings for care approvals/denials.
• Use motivational interviewing to support change.
• Identify barriers and assist with psycho/social and medical needs.
• Identify critical incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25-40% locally based on state requirements.
