Qualifications

• 2+ years in health care with LTSS experience.
• Clinical licensure and/or certification as required by state.
• Valid driver's license and reliable transportation.
• Experience with substance use disorders in some states is a plus.
• Bachelor's degree may be required in certain state contracts.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans, including a waiver service plan with stakeholders.
• Monitors care plans, documents interventions and goal achievement, and suggests changes.
• Promotes integration of services for LTSS, behavioral health, and home/community resources.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and advises on funding sources.
• Facilitates ICT meetings for approval/denial and collaboration.
• Uses motivational interviewing to educate and motivate member change.
• Addresses psycho/social, financial, and medical barriers to care.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• Travel 25-40% based on state/contractual requirements.
