Qualifications

• At least 2 years health care experience including LTSS.
• 1 year of care management or medical/behavioral health setting, or equivalent.
• Valid driver's license and reliable transportation.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Proficiency with Microsoft Office and online portals.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement the care plan with members, caregivers, and providers.
• Monitor care plan effectiveness and document outcomes.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and advise on funding sources.
• Facilitate ICT meetings for approval/denial of services.
• Educate and motivate members using motivational interviewing.
• Identify barriers and provide care coordination for psycho/social, financial, and medical obstacles.
• Identify critical incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25-40% may be required.
