Job summary

Lakeland Care Incorporated in Wisconsin is hiring a Functional Eligibility Specialist to assess eligibility for frail elders, adults with physical or developmental disabilities. You will conduct LTCFS assessments and support care planning, meeting members in person to complete required screens.

The role requires Wisconsin licensure or a bachelor's degree in health or human services, plus LTCFS certification or willingness to complete the training within 30 days. Travel statewide is expected.

Qualifications

• Bachelor’s degree or higher in health or human services or current Wisconsin RN license.
• At least 1 year of professional experience with target populations served by Family Care.
• LTCFS certification current and recent LTCFS experience, or must pass LTCFS training within 30 days of hire.
• Maintain LTCFS certification in good standing by completing required courses and testing.
• Excellent active listening and interviewing skills.
• Strong interpersonal and communication skills; ability to build relationships with stakeholders.
• Ability to travel within Wisconsin and maintain valid driver’s license.

Responsibilities

• Conduct functional assessments and support care planning.
• Meet members in person to complete the Long-Term Care Functional Screen (LTCFS) and other tools.
• Ensure timely and accurate determination of member eligibility in line with DHS contracts.
• Coordinate with the interdisciplinary team to discuss screen findings and quality management.
