Applicants must live within 50 miles of Shawano, hold a relevant degree or RN license, and maintain LTCFS certification. Travel across Wisconsin and to state healthcare facilities is expected.

Qualifikationen

• Bachelor’s degree in a health or human services field or RN license.
• Minimum 1 year professional experience with target groups served by Family Care.
• LTCFS certification and recent LTCFS administration experience or ability to complete LTCFS training within 30 days.
• Maintain LTCFS certification in good standing per DHS requirements.
• Excellent listening/interviewing, and strong verbal/written communication skills.

Aufgaben

• Conduct thorough functional assessments for members.
• Support care planning and LTCFS administration.
• Meet with members in person to complete LTCFS and other assessments.
• Coordinate with Interdisciplinary Team (IDT) members to discuss findings.
• Maintain confidentiality per HIPAA and DHS/State regulations.
