Job summary

Clarest Health OHIO, LLC seeks a Pharmacist for its long-term care pharmacy in Troy, OH. Night shift: seven on/seven off, 8:00 PM–7:00 AM. Salary range $120,000–$137,000 dependent on experience; PharmD or equivalent with active license required.

You will review, verify, and dispense prescriptions, collaborate with technicians, ensure safety and regulatory compliance, and support complex patient care in LTC settings.

Qualifications

• PharmD or equivalent degree in Pharmacy; active state license in good standing.
• Experience with intravenous admixtures and therapy desired.
• Minimum 1 year of pharmacist experience in a healthcare setting.
• Long-Term Care pharmacy experience preferred.
• Familiarity with DocuTrack/Frameworks systems preferred.

Responsibilities

• Process new patient referrals, prepare, dispense, and monitor medications.
• Collaborate with Pharmacy Technicians during preparation and dispensing.
• Provide patient follow-up and monitor labs; liaise with physicians on dosing.
• Impute information accurately and efficiently.
• Ensure compliance with local, state, and federal regulations.
• Support safety practices and enforce policies and procedures.
• Adhere to company and departmental policies.
• Comply with applicable state and federal pharmacy regulations.
• Maintain customer focus and professional relationships with patients and staff.
• Follow Remedi’s Code of Conduct and report violations.
