Job summary

Medication Management Partners is seeking a Full-Time LTC Staff Pharmacist for our Evening Shift. You will review medications, ensure accuracy, and support a fast-paced LTC environment outside traditional retail.

This role reports to the Pharmacist Supervisor and follows a 7 on / 7 off schedule, with specific daily hours. A $5,000 sign-on bonus is offered, along with competitive pay and benefits.

Qualifications

• Illinois Pharmacist License in good standing.
• Long-term care pharmacy experience preferred.
• FrameworkLTC knowledge a plus.
• Ability to multitask and meet deadlines while maintaining professionalism.

Responsibilities

• Interpret and verify prescription orders.
• Review and confirm prescription entry in PV1 system.
• Perform prospective drug utilization reviews.
• Final check of dispensed products (PV2).
• Communicate with medical and nursing staff via electronic, fax, and oral channels.
• Achieve productivity and quality targets.
• Assist with immunization clinics at community partner facilities as needed.
• Model CORE Values and adhere to Behavior Standards.
