Job summary

Medication Management Partners is seeking a Full-Time LTC Staff Pharmacist for our Evening Shift. You will review medications, ensure accuracy, and support a fast-paced LTC environment outside traditional retail.

This role reports to the Pharmacist Supervisor and follows a 7 on / 7 off schedule, with specific daily hours. A $5,000 sign-on bonus is offered, along with competitive pay and benefits.

Qualifications

• Illinois Pharmacist License in good standing.

Responsibilities

Join our growing team at Medication Management Partners! We are currently seeking a Full‑Time LTC Staff Pharmacist for our Evening Shift team. If you are looking for a fast‑paced, collaborative pharmacy environment outside of traditional retail, this could be a great opportunity for you.

Shift

This position follows a 7 on / 7 off schedule, with a Monday‑Friday shift from 11:30 AM to 11:00 PM and a Saturday‑Sunday shift from 8:00 AM to 7:30 PM. This is followed by 7 consecutive days off, before repeating.

Role Description

As a Staff Pharmacist, you are responsible for the accurate and timely review of medications being processed and dispensed at MMP. We are seeking a highly motivated and collaborative individual with a proven track record, experience, and industry knowledge to scale MMP’s operation and execute our transformation from a regional to a national LTC pharmacy provider.

This position reports to the Pharmacist Supervisor and is critical to the operational success of MMP. The position may require long hours and/or weekend work to fulfill position responsibilities.

Responsibilities (including but not limited to)

• Interpret and identify prescription orders
• Verify prescription order entry in the computer system (PV1)
• Perform prospective drug utilization reviews
• Final check (Inspect) dispensed prescription products (PV2)
• Correspond with medical and nursing staff via electronic, fax, and oral communication
• Achieve productivity and quality requirements
• Assist with immunization clinics at community partner facilities as needed
• Leads by example, exhibiting the CORE Values each day, and adheres to the MMP Behavior Standard
• Encourages teamwork and promotes company philosophy
• Attends required company meetings
• Completes all required courses in adherence with the PRS compliance system
• Is prompt and able to perform the required duties of the position on a regular, predictable basis

Requirements

• Current, and in good standing, Illinois Pharmacist License
• Long‑term care pharmacy experience preferred
• FrameworkLTC knowledge a plus
• Able to multitask and work under deadlines while maintaining a positive and productive demeanor

• Track record of proactively tackling issues in a timely and decisive manner
