Job summary

Sweetwater Care is seeking a Registered Nurse (RN) for a Long Term Acute Care (LTAC) setting with trach/vent experience. You will provide direct patient care, collaborate with multidisciplinary teams, and support rehab goals to improve patient independence and quality of life.

The role requires an RN license, a BSN/ASN, and 1–2 years of acute or respiratory nursing experience, including trach and ventilator care, plus BLS/ACLS certification.

Qualifications

• Must hold a current RN license in the applicable state.
• Associate or BSN degree required.
• Minimum 1–2 years in acute or respiratory nursing.
• Experience with trach and ventilator care required.
• BLS/ACLS certification; CRRN preferred.

Responsibilities

• Provide comprehensive nursing care to LTAC patients recovering from surgery, illness, or injury.
• Manage patients with tracheostomy and ventilator support, including suctioning and monitoring.
• Assess conditions and develop care plans with physicians and therapists.
• Administer meds and treatments, monitor effects and side effects.
• Assist with ADLs and mobility; educate patients/families on rehab goals and discharge planning.
• Document accurately in electronic health records and participate in quality initiatives.
