Job summary

Nursefinders Staffing Division seeks experienced LTAC Registered Nurses to deliver care for medically complex patients on a per diem basis. This role features 12-hour day or night shifts and a focus on continuity of care in a high-acuity setting.

You will manage vents, tracheostomies, feeding tubes, central lines, wounds, administer meds, document outcomes, and collaborate with physicians and rehabilitation teams to support patient stabilization and transitions.

Qualifications

• Active RN license in the state of practice.
• Minimum of 1 year of LTAC, ICU, Step-Down, or acute care hospital RN experience.
• BLS certification required (ACLS preferred).

Responsibilities

• Provide comprehensive nursing care to patients with complex, long-term medical needs.
• Perform detailed patient assessments and continuous monitoring.
• Manage ventilators, tracheostomies, feeding tubes, central lines, and complex wounds.
• Administer medications, IV therapies, titratable drips, and blood products per provider orders.
• Monitor and respond to changes in patient condition promptly.
• Collaborate with physicians, respiratory therapists, case management, and interdisciplinary teams.
• Support patient stabilization, recovery, or transition to the next level of care.
• Educate patients and families on disease processes and long-term treatment plans.
• Maintain strict adherence to infection control and patient safety protocols.
• Document assessments, interventions, and outcomes accurately in the medical record.
