Job summary

Vibra Permanent Staffing is seeking an experienced Registered Nurse Case Manager for a direct hire, full-time opportunity with one of our LTAC partners. This role focuses on coordinating care for medically complex patients in a specialty LTAC setting.

You will collaborate with physicians and an interdisciplinary team, monitor progress, administer treatments, educate patients and families, and advocate for long-term health and recovery while ensuring compliance with all policies and regulatory

Qualifications

• Active RN license in the state or compact state.
• 1+ year of Case Management experience preferred.
• BLS required; ACLS within 90 days of hire.
• Strong clinical judgment and a commitment to excellent patient care.
• Compassionate, team-oriented mindset.

Responsibilities

• Provide direct care to medically complex patients.
• Collaborate with physicians and an interdisciplinary team to coordinate individualized care plans.
• Monitor patient progress, administer medications and treatments, and document clinical findings.
• Educate patients and families, and advocate for long-term health and recovery.
• Maintain compliance with all policies, standards, and regulatory guidelines.
