Job summary

Alvita Care is seeking experienced, Licensed Practical Nurse (LPN) for Central and Northern Connecticut. Shifts range from 6-, 8-, to 12-hour blocks with Day and Night options. Per Diem, Part-time or flexible schedules to fit your current commitments.

Shifts provide 1:1 in-home care. CT LPN licensure and BLS/ACLS/CPR certification required. Minimum of one year in the role, patient-focused care, and strong organizational skills are essential.

Qualifications

• CT LPN licensure is required.
• BLS, ACLS, or CPR certification required.
• Minimum of one year of LPN experience.
• Ability to put patients at ease and explain physician's instructions.
• Ability to multi-task with excellent organization skills.

Responsibilities

• Consult with management on complex cases and coordinate care.
• Monitor/assess patients’ condition and the care plan effects.
• Oversee care provided by LPNs and paraprofessionals.
• Ensure proper discharge planning and documentation in systems.
• Communicate with physicians and families as needed.
• Assist patients with activities of daily living.
• Travel to patient homes for in-person visits.

QUALIFICATIONS:

• CT LPN licensure.
• BLS, ACLS, or CPR certification.
• Minimum of One (1) year demonstrated experience as an LPN
• Ability to put patients at ease and explain physician's instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and
  socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently
• Valid driver's license or NYS Non-Driver photo ID card, required
• Drivers preferred!

ESSENTIAL DUTIES & RESPONSIBILITIES:

• Consult with Management on complex/unique cases
• Monitor/Assess patients/clients' condition, effects of the care plan,
• Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manages, plans, coordinates and executes the patient's safe discharge from service.
• Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.
• Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
• Assists patients with activities of daily living
• Documents content and process of all services provided in internal system properly.
• Transport/utilize medical equipment to and from clients/patients' home/facilities. Travel to patient/clients' homes and/or other facilities.
• Perform other related duties and participates in special project, as assigned.
