Job summary

Alvita Care is seeking experienced, per diem Registered Nurse (RN) to cover Fairfield, New Haven & Litchfield Counties with 6- to 12-hour shifts. Day and night options, 1:1 home care, and flexibility to work part-time or around existing schedules are offered.

The role requires CT RN licensure, at least 1 year RN experience, and BLS/ACLS or CPR certification. Driving is preferred; bilingual candidates are a plus and travel to patient homes is expected.

Qualifications

• Must hold CT RN licensure and maintain CPR/BLS/ACLS certification.
• Minimum 1 year RN experience.
• Ability to explain physician instructions clearly to patients.
• Strong multitasking and organizational skills.
• Experience with diverse backgrounds.
• Valid CT driver’s license or CT Non-Driver ID; driving preferred.
• Bilingual candidates a plus.

Responsibilities

• We have clients throughout CT requiringshifts from 6hrs - 8hrs to 12hrs

QUALIFICATIONS:

• CT RN licensure.

• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.

ESSENTIAL DUTIES & RESPONSIBILITIES:

• Assessments needs of the clients to develop a plan of care
• Consult with Management on complex/unique cases
• Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients'/patients/.
• Monitor/Assess patients'/clients' condition, effects of the care plan,
• Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manages, plans, coordinates and executes the patient's safe discharge from service.
• Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.
• Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
• Assists patients with activities of daily living
• Documents content and process of all services provided in internal system properly.
• Transport/utilize medical equipment to and from clients'/patients' home/facilities. Travel to patient/clients' homes and/or other facilities.
• Perform other related duties and participates in special project, as assigned.
