Job summary

Lancaster Nursing & Rehabilitation Center is seeking an experienced LPN Wound Care Coordinator to join our nursing team. This full-time, hourly clinical role focuses on coordinating and supporting the facility’s wound care program, working closely with the Director of Nursing and the interdisciplinary team to promote skin integrity and resident comfort.

Typical hours are 8:00 AM–4:00 PM, with flexibility as needed.

Qualifications

• Graduate of an accredited school of nursing.
• Active LPN license in Pennsylvania.
• Wound care experience or certification is a plus.
• Experience in long-term care preferred.
• Strong documentation and wound care knowledge.
• Ability to work independently and with an interdisciplinary team.

Responsibilities

• Coordinate and support the facility's day-to-day wound care program in accordance with nursing standards and regulations.
• Complete accurate wound documentation, including nursing notes and wound tracking.
• Perform skin and body assessments on new admissions as required by policy.
• Assess residents at increased risk for skin breakdown and implement prevention interventions.
• Perform wound treatments within the LPN scope of practice and ensure scheduled treatments are completed.
• Photograph pressure areas and wounds per protocol.
• Monitor residents for changes in skin integrity and communicate concerns promptly.
• Participate in weekly wound care meetings and QA activities related to resident care.
• Assist with staff education and in-service training on wound care and infection control.
• Maintain compliance with infection-control, PPE, hand hygiene, isolation, and universal precautions.
• Collaborate with nursing staff and other departments to ensure timely resident care.
• Maintain accurate clinical documentation and records related to wound care.
• Provide direct nursing care as necessary.
• Participate in continuing education and maintain licenses and certifications.
• Perform additional related nursing duties as assigned.
