Qualifications

  • Current, active RN or LPN license in the United States.
  • Wound care experience preferred.
  • Strong knowledge of skin integrity and wound protocols.

  • Effective communication with physicians, families, and staff.

Responsibilities

  • Coordinate and administer physician-ordered wound care and treatments.
  • Perform skin assessments on all new admissions and ongoing residents.
  • Monitor, document, and photograph wound progress and interventions.
  • Communicate changes in wound condition to physicians, nursing leadership, and families as appropriate.
  • Maintain accurate and timely wound documentation and treatment records.
  • Participate in care planning, MDS support, and interdisciplinary collaboration related to wound care.
  • Assist with tube feedings and related treatments as assigned.
  • Order wound care supplies and maintain inventory levels.
  • Participate in quality assurance, survey readiness, and audits related to skin integrity.
  • Provide education and guidance to nursing staff on wound care practices.
  • Complete incident reports and documentation related to skin or treatment concerns.
  • Ensure treatment areas are clean, safe, and compliant with infection control standards.

Full-Time | Part-Time | Day Shift

We are seeking a skilled and detail-oriented Wound / Treatment Nurse (RN or LPN) to coordinate and provide wound care and prescribed treatments for residents. This role is critical to skin integrity, healing outcomes, regulatory compliance, and overall quality of care.

This position is ideal for a nurse who is organized, clinically strong, and passionate about wound management and treatment accuracy.

Responsibilities

Physical Requirements
  • Standing and walking throughout the facility
