Job summary

Coastal Health & Rehabilitation Center is seeking a qualified Wound Care Nurse (RN or LPN) to join our skilled team and provide high-quality wound management for residents. You will liaise with the Wound Specialist Physician, develop individualized treatment plans, and educate staff to promote best practices in wound care.

Qualifications

• RN or LPN with active state licensure.
• Wound Care Certification and CPR required.
• Minimum 1 year wound care experience in healthcare.
• Compassionate, detail-oriented with strong communication.

Responsibilities

• Liaison between Physician and Nursing Staff to ensure seamless communication and care coordination.
• Develop and align Wound Care Program objectives with guidelines and best practices.
• Provide staff development by delivering educational programming on wound care knowledge.
• Assist in implementing wound care policies and ensuring regulatory compliance.
• Collaborate with Interdisciplinary Team to address care aspects contributing to wound healing.
• Assess, treat, and monitor residents' wounds as ordered by the physician.
• Create individualized treatment plans to promote healing and outcomes.
• Maintain ongoing communication with physicians on resident care updates.
• Assist in regulatory surveys and inspections as directed by leadership.
• Identify opportunities to improve wound care processes and resident outcomes.
