Job summary

Chelsey Park Health and Rehabilitation is seeking a licensed Practical Nurse (LPN) for full-time day-to-day patient care in Georgia. You will monitor skin conditions, administer medications, and document patient progress in coordination with the nursing team.

The role emphasizes effective communication with physicians and timely reporting to the Director of Nursing. Georgia license is required; prior wound care experience is a plus.

Qualifications

• Nursing Degree from an accredited college or university or is a graduate of an approved LPN program.
• An active LPN License to practice in the State of Georgia.

Responsibilities

• Monitor any skin problems that the patients may have and treat as ordered by physician.
• Administer and record all treatment medications.
• Chart information regarding patient’s progress, ailments, medication, symptoms and observations.
• Provide to the Director of Nursing appropriate reports/documentation to support departmental updates, corrective actions, and others as necessary.
• Identify, manage and treating acute and chronic wounds and skin lesions (e.g., pressure ulcers, skin tears, skin infections).
• Maintain effective lines of communication with attending physicians.
