Job summary

Qualifications

Responsibilities

• Assess resident conditions to implement intervention and develop Care Plans.
• Make rounds with physician; observe and evaluate resident symptoms and responses to treatments.
• Administer medications and coordinate with physicians and nursing staff.
• Document in PCC and maintain accurate records; manage care plans and reports.
• Supervise CNAs and assist residents with daily activities; report incidents promptly.
