Maxim Healthcare in Hebron is seeking Licensed Practical Nurses (LPN) for homecare visits with patients in a Visit Nurse role. Typical visits range 35 minutes to 1 hour and may include med pass, insulin assistance and wound care. Evening and weekend availability is emphasized to support patient needs.

Two tiered scheduling exists: Tier 1 requires 4 days per week with weekend coverage or one weekend day; Tier 2 requires 3 days per week with weekend coverage. A casual/PRN option is also available.

Formación

• Current RN or LPN license for the state of practice.
• Health Certificate (per facility RN contract or state regulation).
• PPD or Chest X-Ray current.
• BLS certification current.

Responsabilidades

• Plan, implement and evaluate client care using the nursing process.
• Initiate care plan updates with prescriber as patient status changes.
• Therapeutic, preventive and rehabilitative nursing actions based on diagnosis and goals.
• Respond to changes in patient condition and notify physician or supervisor.
• Administer medications and treatments; monitor and document effects.
• Review care plans and program protocols; coordinate education and discharge planning.

Pay: $25 - $28 / per visit

Maxim Healthcare in Hebron is hiring for Licensed Practical Nurses (LPN) to work with patients in the homecare setting for a Visit Nurse Position, which includes quick 35 minute - 1 hour visits in the patient's home. Visits can include med pass, insulin assistance, wound care, etc. We are primarily looking for EVENING AND WEEKENDS

Intermittent Nursing Team Tier 1

This nursing team will enjoy consistent weekly scheduling centered around an expected number of visits/days per week.

Expectations

• Be available 4 days per wk.
• Be available E/O weekend or one weekend day per week
• Complete a minimum of 32 visits per wk.
• Compensation discussed on an individual basis.

Intermittent Nursing Team Tier 2

This nursing team will enjoy consistent weekly scheduling centered around an expected number of visits/days per week.

Expectations

• Be available 3 days per wk.
• Be available E/O weekend or one weekend day per wk.
• Complete a minimum of 24 visits per wk.
• Compensation discussed on an individual basis.

There is also a casual/ PRN option with no set availability expectations!

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) for the state in which the nurse practices.
• Current Health Certificate (per facility RN contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
