Iowa Home Care LLC is seeking an LPN to deliver compassionate, physician-directed nursing care in clients' homes. You will conduct home visits, assess conditions, administer treatments, and collaborate with a supportive interdisciplinary team to maintain health and independence.

The successful candidate will have Iowa LPN license and 1–2 years of hospital or home health experience, strong communication skills, and reliable transportation.

Competenze

• LPN license in Iowa.
• 1–2 years of experience in acute hospital or home health care preferred.
• Knowledge of nursing standards as set by the Iowa Board of Nursing.
• Strong communication skills and commitment to confidentiality.
• Valid driver’s license and reliable transportation.

Mansioni

• Deliver care according to the physician's plan and established nursing standards.
• Conduct home visits and shifts, assessing client conditions, recording vitals, and noting changes.
• Administer nursing procedures, manage medications, and provide treatments per physician orders.
• Promote home safety, infection control, and fall prevention with families.
• Maintain clear communication with the RN Case Manager and team members to coordinate care.
• Document all care delivery accurately and timely.
• Train families in home care practices and collaborate to meet care objectives.
