Job summary

WesleyLife in Des Moines, IA, is seeking an LPN Visit Nurse to join our Home Health team. You will provide hands-on nursing care guided by individualized care plans, helping clients stay safe and independent at home.

You will participate in on-call rotations—including evenings, weekends, and holidays—document visits in the EMR, and collaborate with the care team to maintain high-quality, compassionate care. Full-time Monday-Friday with rotating on-call.

Responsibilities

• Provide hand-on nursing care guided by individualized care plans, ensuring each client receives the support they need to remain safe and independent at home.
• Monitor changes in health and well-being, promptly reporting updates to the care team and ensuring continuity and quality of care.
• Document your visits in the electronic medical record (EMR) with accuracy and clarity, helping the entire team stay connected and informed.
• You’ll participate in on-call rotations, including evenings, weekends, and holidays—providing urgent support with calm professionalism when it’s needed most.
• You’ll join nursing case conferences, attend team meetings, and complete ongoing training, contributing to a collaborative environment that strengthens care quality and professional growth.
