Job summary

WesleyLife Home Health in central Iowa is seeking an RN with experience in home health, wound care, or infusion therapy to join our mission-driven team in supporting older adults at home.

Qualifications

• Active RN license in Iowa.

Responsibilities

• Perform comprehensive nursing assessments during in-home visits (recertifications, visits, and care plans).
• Evaluate client progress and response to treatment to adjust care plans.
• Counsel clients and families on health conditions, medications, and self-care.
• Provide guidance to empower families and ensure safety at home.
• Maintain communication with the Nurse Case Manager and updates to orders.
• Collaborate with the interdisciplinary team for coordinated care.

Responsibilities

As a Visit Nurse (RN), you will play a key role in supporting the health, safety, and independence of the clients we serve. Your responsibilities will include:

Clinical Visits & Assessments

• Perform comprehensive nursing assessments, including recertifications, resumptions of care, discharge assessments, and routine in-home visits.
• Evaluate client progress, response to treatment, and ongoing needs to ensure high-quality, person-centered care.
• Counsel clients and their families on health conditions, care plans, medication management, and other related needs.
• Provide clear, compassionate guidance that empowers families and supports safety and independence at home.
• Maintain ongoing communication with the Nurse Case Manager regarding updates to healthcare provider orders, changes in client condition, incident reports, etc.
• Collaborate with the interdisciplinary team to ensure seamless, coordinated care.
