Job summary

The Plaza Rehab and Nursing Center in New York is seeking an experienced LPN to oversee resident care on a nursing unit. The nurse will lead nursing assistants, perform rounds, and ensure medications and treatments follow physician orders with compassionate, patient-centered care.

You will collaborate on care plans, support physicians, and document activities, while supporting QA processes and staff development.

Qualifications

• Licensed Practical Nurse (LPN) licensed in the state of employment.
• Minimum one year of education or experience in geriatric nursing or rehabilitation.
• CPR certification.
• Ability to read, write and comprehend English.

Responsibilities

• Holds daily shift report with nursing assistants and reviews assignments before shift end.
• Mounds rounds on assigned residents at least every two hours.
• Administers medications and treatments per physician orders; orders medications.
• Collaborates with Care Plan Coordinator to develop and update nursing care plans monthly or as condition changes.
• Assists attending physician with rounds and special procedures as needed.
• Transcribes, documents and implements physician orders.
• Notifies physician and family of condition changes; informs supervisor when appropriate.
• Conducts yearly evaluations of nursing assistants and documents performance and wage decisions.
• Documents disciplinary concerns for nursing assistants on designated shifts and recommends action.
• Completes routine documentation and auditing as required.
• Participates in hiring, interviewing and orientation of nursing assistants.
• Assists in Quality Assurance and Performance Improvement (QAPI) process.
