Job summary

Buena Vida Rehabilitation and Nursing Center in New York is seeking an LPN Supervisor to oversee nursing care on assigned shifts, providing leadership to LPNs, CNAs, and other staff while delivering hands-on care.

Under the Director of Nursing, you will supervise daily activities, ensure adherence to care plans, administer medications, document accurately, and coordinate with the healthcare team to maintain high-quality resident care.

Qualifications

• LPN licensed by the State NYC.
• Must have 1-2 years of Long Term Care (LTC) experience.
• Must have a reliable source of transportation.
• Must have a valid driver's license and automobile insurance.

Responsibilities

• Provide direct nursing care per physician orders, care plans and facility policies.
• Administer medications, treatments, and procedures within LPN scope.
• Monitor resident condition and promptly report changes to staff.
• Lead and supervise LPNs, CNAs, and other direct care staff on shift.
• Ensure accurate documentation of charting, meds, and care activities.
• Coordinate care with the nursing director and interdisciplinary team.
