Qualifications

• Must have knowledge of disease processes and community health.
• Must operate with cultural sensitivity when working with diverse populations.
• General computer skills with experience using EHR systems.

Responsibilities

• Coordinate and schedule walk-in and provider appointments.
• Triage patients using established protocols based on symptoms.
• Document assessments and care plans in the EHR.
• Support providers with patient communications and referrals.
• Monitor patient flow and ensure timely services.
• Assist with prescriptions refills per protocol.
