Maxim Home Healthcare is seeking a Licensed Practical Nurse (LPN) to provide high-quality home-based care. You will work independently in patients' homes under a clinical manager, delivering comprehensive care and education to patients and families.

Qualifications

• Active LPN license.
• Ability to travel to patients' homes.
• Experience in home healthcare preferred.

Responsibilities

• Assess, plan, and implement patient care under the supervision of a clinical manager.
• Educate patients and families on disease processes, procedures, and medications.
• Manage wound care, tracheostomy, g-tubes, catheters, ventilators, and medication administration.
• Create customized care plans to accommodate patient needs and family concerns.
