Elm Terrace Gardens in Lansdale, PA seeks a compassionate Licensed Practical Nurse Supervisor (LPN) to lead the Personal Care team, supervising Med Techs, CNAs and Direct Care Workers to ensure residents receive high-quality care.

The role involves assessing residents, monitoring health status, and administering medications while maintaining accurate documentation and collaborating with the healthcare team. Bi-weekly 7p-7a shifts and a supportive, compliant work environment are provided.

Qualifications

• At least 2 years of supervisory experience in a personal care setting or skilled nursing.
• Valid PA LPN license.

Responsibilities

• Supervise Med Techs, CNAs and direct care workers to ensure quality care.
• Assess, admit and discharge residents as needed.
• Monitor health status and vital signs; document changes.
• Administer medications as prescribed.
• Maintain documentation and care plans with the healthcare team.
• Ensure a safe, clean environment and promote independence.
