Job summary

Elm Terrace Gardens in Lansdale, PA is seeking a compassionate Licensed Practical Nurse (LPN) Supervisor for the Personal Care unit. The role supports Med Techs, CNAs, and Direct Care Workers on a 7pm-7am schedule, two days per week with every other weekend, within a LifePlan Community.

The position emphasizes supervising daily care, monitoring residents, and coordinating with the healthcare team to maintain safety and comfort for residents, with an emphasis on dignified independent living.

Qualifications

• Valid and current Licensed Practical Nurse (LPN) license in Pennsylvania.
• At least 2 years supervisory experience in a personal care or skilled nursing setting.
• Strong interpersonal and communication skills.
• Compassionate, patient, and respectful toward residents.
• Ability to follow detailed instructions and safety protocols.
• Good organizational skills and attention to detail.
• Ability to work independently and as part of a team; ability to lead a team and make emergent decisions.
• Physical ability to assist with mobility and caregiving tasks.

Responsibilities

• Supervise care team (Med Techs, CNAs, Direct Care Workers) to ensure high-quality resident care.
• Perform assessments, admissions, and discharges as needed.
• Monitor and record residents' health status and vital signs, noting changes.
• Administer medications and treatments per physician orders.
• Conduct cart audits, RASP documentation, and DME records.
• Support residents with activities of daily living, promoting independence and dignity.
• Maintain accurate documentation of care provided and resident progress.
• Collaborate with healthcare team to develop and implement individualized care plans.
• Ensure a safe, clean, and comfortable environment for residents.
