Qualifications

• High School Diploma or equivalent required;
• Associate's Degree preferred for advancement;
• Valid NYS LPN license required;
• Current CPR certification required;
• Supervisory or leadership experience preferred;
• Experience in long-term care or skilled nursing preferred;
• Strong clinical assessment, communication, and organizational skills.

Responsibilities

• Supervise nursing staff and support the delivery of high-quality resident care
• Administer medications, treatments, and nursing services per physician orders and policies
• Monitor resident conditions with appropriate interventions and documentation
• Collaborate with residents, families, physicians, and interdisciplinary team to support care plans
• Provide coaching, training, and mentorship to nursing staff
• Ensure compliance with regulations, infection control, and facility policies
• Maintain accurate documentation of resident care and outcomes
• Address clinical concerns, staffing issues, and resident care needs for continuity of care
• Perform additional duties as assigned

• Ensure compliance with state and federal regulations, infection control practices, and facility policies
• Maintain accurate documentation of resident care, treatments, and clinical outcomes
• Address clinical concerns, staffing issues, and resident care needs to ensure continuity of care
• Perform additional duties as assigned

REQUIREMENTS:

• High School Diploma or equivalent required; Associate's Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Current CPR Certification required
• Supervisory or leadership experience preferred
• Experience in long-term care or skilled nursing preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to lead, mentor, and support nursing staff
• Strong problem-solving abilities and attention to detail
• Commitment to providing compassionate, resident-centered care
