Job summary

Southcoast Health is hiring an LPN Staff for Primary Care – Swansea, MA. The role involves direct patient care, including medication administration and teaching, in a community-focused health system.

Requirements include a Massachusetts/Rhode Island LPN license and BLS, with prior EHR experience encouraged. A 24-hour schedule and a $3,000 sign-on bonus are offered to eligible candidates.

Qualifications

• Graduation from an accredited school of practical nursing or equivalent is required.
• Experience working with Electronic Health Record (EHR) is highly desirable.
• One year or more experience in related setting preferred.
• Current LPN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is also required.
• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Responsibilities

• Direct patient care including medication administration, patient and family teaching, and testing/procedures.
• Use an Electronic Health Record (EHR) and related software to update or access patient information.
• Provide care appropriate to the age group regularly served.
• Collaborate with nursing and patient care teams to maintain professional standards.

We are searching for a talented LPN Staff

• $3,000 new hire sign-on bonus for this position. (rehires termed more than 1 year also eligible)*

Hours: 24hrs

Shift: Day shift; rotating start times 8:00am - 4:30pm and 8:30am - 5:00pm

Responsibilities

Position reports to the Director, Manager or designee. Perform a variety of duties involving direct patient care, such as medication administration, patient and family teaching, and testing/procedures in accordance with organizational policies, procedures, and protocols and within the scope of practical nursing. Utilize an Electronic Health Record (EHR) and other related software to update or access patient information as appropriate. Has knowledge and skills necessary to provide care, which is appropriate to the age group of patients regularly served. Works cooperatively with ancillary nursing and other patient team personnel in maintaining standards for professional nursing practice.
