Job summary

Southcoast Health in Wareham, MA is seeking an LPN for the Float Pool in Primary Care. This 40-hour-per-week role offers a day shift with two options: 8:00am–4:30pm or 8:30am–5:00pm, in Wareham, MA. Eligible for a $5,000 new hire sign-on bonus for this position.

You will provide direct patient care, administer medications, and teach patients and families. Occasional travel to SPG Primary Care sites may be required to cover staffing shortages.

Qualifications

• Direct patient care including medication administration and patient/family teaching.
• Experience with Electronic Health Records (EHR) is preferred.
• Six months+ related work experience is desirable.
• Current LPN license in MA and/or RI is required.
• BLS certification required.
• Vaccination for influenza and COVID-19 or approved exemption is required.

Responsibilities

• Perform a variety of duties involving direct patient care per policies and procedures.
• Facilitate efficient flow of patients through the clinic or medical office.
• Communicate test results, schedules, and follow-up instructions to patients under supervision.
• Utilize EHR and related software to access or update patient information.
• Travel to SPG Primary Care sites as needed to cover staffing shortages.

Responsibilities

Position reports to the Director of Clinical Operations/designee. Perform a variety of duties involving direct patient care, such as medication administration, patient and family teaching, rooming, and procedures in accordance with organizational policies and procedures and within the scope of practical nursing. Facilitate efficient flow of patients through the clinic or medical office. May be responsible for telephone communication with patients including communication of test results, surgery schedules, procedures and providing appropriate counsel and follow-up under the direction of a designated supervisor. Utilise an Electronic Health Record (EHR) and other related software to update or access patient information as appropriate. Has knowledge and skills necessary to provide care, which is appropriate to the age group of patients regularly served. Position requires nurse to travel to SPG Primary Care sites where there may be a staffing shortage, ensuring patient care is delivered safely and appropriately.

Qualifications

• Graduation from an accredited school of practical nursing or equivalent is required.
• Experience working with Electronic Health Record (EHR) is highly desirable.
• Six months or more experience in related setting preferred.
• Current LPN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is also required.

• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

• Compensation: Pay rate will be determined based on level of experience.

Pay Range

USD $28.66 - USD $44.60 /Hr.
