Job summary

Advocate Health is seeking a Licensed Practical Nurse (LPN) to provide compassionate care across Skilled Nursing, Assisted Living, and Memory Care at Carlyle Place in Northwest Macon, GA. The role supports resident-focused plans under RN supervision and emphasizes safe, quality outcomes.

You will work full-time day shifts (7a-7p), including every other weekend, in a complex senior living environment with growth opportunities and comprehensive benefits.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted Georgia LPN license (or compact license eligible for Georgia).
• Current CPR certification or ability to obtain within 60 days of hire.
• Basic Life Support (BLS) certification required per policy.
• Skilled nursing, long-term care, assisted living, or memory care experience preferred.

Responsibilities

• Provide nursing care in Skilled Nursing, Assisted Living, and Memory Care settings.
• Monitor resident conditions and report changes to the healthcare team.
• Assist with assessments, care planning, treatments, and procedures.
• Administer medications and treatments within scope of practice.
• Educate residents and families regarding care plans and treatments.
• Document assessments, interventions, and resident responses accurately.
• Collaborate with interdisciplinary team to support positive resident outcomes.
• Ensure care is delivered safely, efficiently and in accordance with regulatory requirements.
