Job summary

Cornerstone Management is seeking an LPN in Minnesota to support planning, coordination, and management of assisted living services under RN supervision. You will ensure health and safety, monitor residents, and assist with medication management in compliance with regulations.

Ideal candidates have a MN LPN license, 3+ years in the field, and strong communication and leadership skills to work with residents, families, and the RN Supervisor.

Qualifications

• Must be licensed in MN as LPN and maintain current, unencumbered licensure.
• At least 3 years experience in the related field.
• Sensitivity and aptitude for working with the elderly.
• Ability to communicate effectively with residents, families and RN Supervisor.
• Leadership and supervisory skills.
• Ability to prioritize and organize work; irregular hours including weekends/evenings.

Responsibilities

• Provide nursing support in accordance with Assisted Living regulations and community standards.
• Monitor resident health, update care plans, and coordinate with RN and physicians.
• Assist with medication management and coordinate renewals with pharmacies.
• Maintain resident records and nursing notes; ensure privacy and confidentiality.
• Collaborate with RNs and care staff to deliver resident-centered care; participate in team meetings.
