Great Lakes Management in Minnetonka, MN is seeking an LPN to join our assisted living team. You will monitor residents under state regulations, maintain care plans, and coordinate with physicians and the care team to ensure safe, compassionate services.

Qualificações

• Keeps an active LPN license as per MN regulations.
• Minimum of one year experience in home health, assisted living, or long-term care preferred.

Responsabilidades

• Assists residents in meeting admission criteria and monitors needs, recommending placement changes as needed.
• Ensures care plans are up to date per state regulations.
• Maintains resident charts and progress notes.
• Communicates changes in residents' needs to physicians, staff, and family.
• Coordinates resident services with other providers.
• Oversees medications and renewals; follows physician guidance.
