Job summary

Montefiore Medical Center is seeking an LPN for the Montefiore School Health Program at PS 198 in Bronx, NY. This AWS role supports student health in a school-based setting, focusing on preventive care, chronic disease management, and urgent clinical needs.

You will collaborate with RNs and school staff, provide direct patient care to pediatric and adolescent students, administer medications, collect specimens, and educate families.

Qualifications

• High School Diploma or GED minimum.
• Current New York State LPN License & Registration.
• Basic Life Support (BLS) Certification.
• Infection Control Certificate.
• Effective written, oral, and interpersonal English communication skills.
• Basic data entry and computer retrieval skills.
• Must complete fingerprinting for Department of Education (DOE) and Justice Center Security Clearance.
• Occupational Health Services (OHS) Annual Assessment & Fit Test.

Responsibilities

• Direct patient care to pediatric/adolescent students under RN supervision.
• Safely administer medications, treatments, and procedures within LPN scope; venipuncture where applicable.
• Deliver patient/family education and discharge instructions tailored to developmental needs.
• Maintain infection control, safety, and disaster policies; document care in the EHR.
• Collaborate with RNs and school staff to manage patient needs and daily workflow.
