Job summary

CenterWell Home Health is seeking a Patient Services Coordinator-LPN to coordinate scheduling of visits for patients and communicate with field staff, patients and physicians to maintain high-quality care coordination. The position is onsite in Kankakee, IL, full-time, Monday to Friday, 8am-5pm, offering a comprehensive benefits package through Humana/CenterWell.

Qualifications

• Must be a Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
• At least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Excellent communication skills, the ability to interact with a diverse group, strong organizational skills, and the ability to manage and prioritize multiple assignments.

Responsibilities

• Manages schedules for all patients and reassigns as needed.
• Initiates infection control forms and forwards to HRD.
• Back-up during lunch hours and busy times; supports field staff calls.
• Maintains hospitalization log and shares notes with staff.
• Ensures appropriate scheduling of skilled nurse/injection visits and add-ons.
• Runs scheduling reports and prepares weekly agent schedules.
• Verifies visit notes in scheduling console.
• Assists with internal transfers of patients between branches.
• Receives lab reports and routes to medical records; initiates infection reports.
• May perform patient visits and participate in on-call rotation.
