Job summary

Angels Care Home Health is seeking an LPN Scheduler to manage patient visit scheduling using Home Care Home Base. You will document schedules accurately and collaborate with management and the interdisciplinary team to coordinate patient care.

The role requires an LPN/LVN license, 1 year of home health experience, and strong computer and communication skills to support patients, staff and referrals in a fast-paced environment.

Qualifications

• Minimum 1 year of home health experience.
• Graduate of an accredited school of practical/vocational nursing.
• Current state LVN/LPN license and driver’s license.

Responsibilities

• Schedule and document patient schedules in Home Care Home Base.
• Coordinate care with management and interdisciplinary team.
• Communicate effectively with patients, staff and referral sources.
• Respond to agency and patient needs professionally.
• Demonstrate commitment, growth and competency; promote agency policies.
