Angels Care Home Health is seeking an LPN Scheduler to manage patient visit schedules using Home Care Home Base. You will document schedules promptly and collaborate with management and the interdisciplinary team to ensure coordinated care.

The role requires strong communication, organizational skills, and the ability to respond to agency and patient needs professionally. A nursing background with LPN/LVN licensure is essential for this position.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LVN/LPN.
• Current State’s Driver’s License.
• Reliable transportation with valid auto liability insurance.

We want you to grow your career with us!Job Summary:The primary function is to process all aspects of scheduling nursing visits via Home Care Home Base software system.Functions:Schedule and appropriately document patient schedules in a timely manner.Participate in coordinating care with management and patient interdisciplinary team.Provide effective communication to patients, staff members, other health care professionals and referral sources.Respond to agency and patient needs in a professional and creative manner.Demonstrate commitment, professional growth and competency.Promote Agency philosophy and administrative policies to ensure quality of care.Skills & Experience:Minimum 1 year of home health experience.Excellent computer skills required.Excellent interpersonal and organizational skills.Basic knowledge of scanner/fax/copier equipment.Phone etiquette skills required.Ability to take direction with a multitude of tasks.Typing ability 40-50 WPM.Knowledge of Medicare regulatory guidelines.Qualifications:Graduate of an accredited school of practical/vocational nursing.Current state license as an LVN/LPN.Current State’s Driver’s License.A reliable source of transportation and valid/current auto liability insurance is required.We are an equal opportunity employer.
